Active Ageing - South Glos

Overview

This service is only available to patients who are registered with a South Gloucestershire GP Practice.

Health Visitors for the Active Ageing Service aim to focus on health and well-being rather than disease. We work within the Public Health agenda by promoting health, and attempting to reduce health inequalities. Health Visitors use a partnership model and an empowerment approach to increase acceptability with clients.

At present, any Health and Social care professional within South Gloucestershire can refer people over 70 years old.

Referral Criteria

Criteria for appropriate referrals:

  • People who may be at risk of falling
  • People who may be at risk of malnutrition, low mood or fading memory
  • People who have recently been bereaved
  • People who are socially isolated
  • People who are living in inappropriate or unsafe housing
  • People who self-refer will be accepted on a case by case basis

Referrers should email the completed Active Ageing Referral Form to sirch.southglosspa@nhs.net

Referrals are not accepted if:

  • There is regular Community Matrons and District Nurse input
  • Patient is living in a Nursing Home or in a Supported Living environment e.g. Extra Care Housing
  • Patients have long term medical conditions currently being managed by other medical services for example community rehab teams
  • Patients under 70 years of age

 This is not an emergency service and patients with long-term conditions are not monitored.
