Joint Formulary Paperwork
In some circumstances, completion of a shorter ‘New Drug Request Short Form’ is appropriate. Please contact the BNSSG Formulary Team to discuss if a short form is suitable prior to completing the application. Examples of when a ‘short form’ may be suitable include:

New formulation of a medicine that is already included in the formulary for the same indication as formulary use

New biosimilar of a medicine that is already included in the formulary for the same indication as formulary use

New insulin or inhaler device of a medicine that is already included in the formulary

Please send completed paperwork to BNSSG.formulary@nhs.net.
Applications from clinicians based in Secondary Care / Mental Health or Community Services should be sent to the relevant pharmacist within their organisation e.g. Trust Formulary Lead before submitting to the BNSSG Formulary Pharmacists.
