Datix Concerns Reporting

Overview

The BNSSG Quality Team has been working towards harmonising the way clinicians inform the ICB of issues regarding the quality of patient care across BNSSG. In Bristol this was previously via the ‘contact us’ service, in North Somerset the ‘Datix reporting tool’ and in South Gloucestershire the ‘Quality Portal’.

The process involved implementation a new Datix reporting tool, accessed via a web link, and went live in July 2018. The emphasis of the new reporting tool will be the identification of themes affecting the quality or safety of patient care. Themes will be followed up by the quality team with the providers in question and any learning will be shared on the quality pages of the BNSSG website.

Datix system access is not required to submit a concern. Please use the links below to make your report and any feedback will be sent back to the reporter of the event. Themes and trends are sent out quarterly in the GP bulletin

Datix Reporting Top Tips

To support reporters when submitting a Datix, please consider the following top tips:

• Include patient's NHS number and DOB – this is needed when Datix reports are shared with the relevant providers (e.g. acute trust patient safety teams) involved to help them investigate.

• Include the name of the organisation/provider involved e.g. pharmacy name and address (including postcode), acute trust and department if relevant (e.g. respiratory).

• Datix incidents tend to be reported to the ICB retrospectively; please include information about whether the incident has been resolved/patient outcome and/or any learning/changes to internal processes that have already been made because of the incident.

• Include relevant, concise, chronological information which relate to the incident e.g. name of medication involved, indications if relevant. Try to avoid including any irrelevant information. Where possible avoid acronyms.

• If the incident involves a controlled drug, this also needs to be reported to the Controlled Drug Accountable Officer (CDAO) for information and shared learning via https://www.cdreporting.co.uk/ and please acknowledge this has been done in the Datix report.

• If you think helpful, please share relevant clinical letters/discharge documents when you submit the Datix report. You must redact any patient identifiable information.
