The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes or hypertension or both, to manage their weight and improve their health.
The NHS Digital Weight Management Programme is a 12-week online behavioural and lifestyle programme.
People can access it via a smartphone or computer with internet access.
Please let patients know that they must login within 42 days with the chosen service provider or the referral will be rejected - see below.

Referrals should be made via e-RS using the NHS Digital Weight Management template that can be downloaded for EMIS without the need to print and re-scan.
You will need your ODS code (practice code) to access the referral template site.
Referrals via e-RS are done directly by the GP practice via the following route:
