Self-management Hip & Knee pain

ESCAPE-pain

Consider referral to an ESCAPE-pain course [leaflet] in the community for patient to learn how to self-manage and to increase physical activity. What

  • A rehabilitation programme designed to reduce pain and disability for patients with hip and knee OA
  • 10-12 sessions
  • Education and exercise with a motivational interviewing focus
  • Reduces pain
  • Improves physical function
  • Improves the psychosocial consequences of pain

Who

  • Individuals over 45 years of age
  • Have knee and/or hip pain for at least 3 months

Where

If a patient is keen to get going, it may be helpful to signpost them to an on-line provider, https://escape-pain.org/support-tools/ESCAPE-pain-Online so that they can join sooner.

NHS funded ESCAPE pain courses (free of charge for patients) are running in:

  • Brunel Fitness Centre, Speedwell
  • The Leisure Centres run by Circadian Trust in Bradley Stoke, Kingswood, Longwell Green, Thornbury and Yate.
  • Imperial Sports Ground, South Bristol
  • Age UK Somerset, Weston and Worle
  • Greenway Centre, Southmead

There may be other gyms and leisure centres running ESCAPE courses across BNSSG however these are not free for patients and a charge will apply.

Only if, for medical reasons, the patient cannot access the courses run by a community provider, then they can be referred to an ESCAPE-pain course run by an NHS provider. Refer by following the normal pathway to physiotherapy, requesting attendance at an ESCAPE-pain course.

NHS providers are also running courses at the following locations:

  • North Bristol Trust- Southmead Hospital
  • University Hospitals Bristol and Weston- Bristol Royal Infirmary
  • University Hospitals Bristol and Weston- Weston Hospital
  • Sirona- Cossham Hospital
  • Sirona- Knowle West Health Park
  • Sirona- Marina Health Centre

How

Please use the referral form in EMIS Resource Publisher. Patients can also self-refer onto a course by contacting the community provider.

Onward referral

If the patient continues to have difficulty following a course with an NHS provider and a referral to physiotherapy is needed, the exercise instructor will complete a physiotherapy referral form and send directly to physiotherapy for triaging. An update will also be sent to the GP Surgery for information only.

Poster

Nuffield - Joint pain programme

Patients suffering from osteoarthritis, musculoskeletal issues or joint pain can be referred to a Nuffield Health 12 week programme designed to help relieve pain and improve quality of life. An option for patients looking for an alternative solution to medication.

For £2 per session, patients will receive help and advice with nutrition, sleep and overall wellbeing, plus obesity and chronic pain support where necessary. It starts and ends with a free Health MOT, and over the 12 weeks the patient will attend twice weekly sessions held by a trained fitness adviser. (£48 per course)

All patients will undergo an initial assessment for suitability of the programme. If the patients osteoarthritis is considered too severe by the joint pain experts, they may not be eligible to participate in the programme.

The programme takes place at: Nuffield Health Bristol, Fitness & Wellbeing Gym, 83 Queens Road, Clifton, Bristol.

One You - MSK Health Optimisation Pilot

One You South Gloucestershire resumed delivery of the MSK Health Optimisation Pilot effective 01 April 2021.

Who? A free healthy lifestyles and wellbeing service for anyone (>18) who is a South Gloucestershire resident or registered with a South Gloucestershire GP practice.

What?  Support for your patients to stop smoking and lose weight by helping them to stay engaged and motivated through one-to-one buddy support and goal setting.

Why? To ease the pressures arising for those on long waiting lists for Orthopaedics by encouraging weight loss, being more active and stopping smoking while they are waiting.

How? Once you have identified a patient who has responded positively to your intervention you can refer them to One You via:

  • The referral form in EMIS 
  • OR use the MSK One You Referral Form - South Glos
  • OR for patients with no access to email or the internet please ask them to call 01454 865337 and a One You South Gloucestershire team member will be in touch with them within two working days to discuss their options
