Secondary Care to maintain responsibility for monitoring amber specialist initiated drugs in the cardiovascular section of the Paediatric formulary. A clear management plan detailed in the clinic letter is required to support primary care prescribing. GP Practices should ensure a plan is in place before taking over prescribing responsibility

Diuretics

 

Loop Diuretics

Furosemide (TLS Amber specialist initiated)

  • For liquid use 50mg/5mL strength liquid

Alternative:

Bumetanide (TLS Red)

 

Osmotic Diuretics

Mannitol 20% infusion (TLS Red)

 

Potassium-sparing Diuretics

Amiloride (TLS Amber - SCP needed)

 

Thiazides and Delated Diuretics

Alternative:

Chlorothiazide (TLS Amber - SCP needed)

Bendroflumethiazide 2.5mg tablets (TLS Amber Specialist Initiated)

Specific Indications:

Metolazone (unlicensed) (TLS Red)

  • For use in combination with a loop diuretic

 

Carbonic anhydrase inhibitors

Specific indication:

Acetazolamide 250mg tablets (TLS Amber - SCP needed)
