Any infant or child who is impaired or disordered in one or more areas of development despite intervention from first tier services such as the health visitor.
Children at risk of developmental problems due to their medical history, such as specific genetic disorders or extreme prematurity.
Please collate information from all known health professionals eg Health Visitor, Children’s Centre, Social Worker to include within referral information.
Referral can be made to the team using the Single Point of Entry form. This should also be available as an EMIS template.
