Overview

There are a number of self-help resources available to Self Gloucestershire residents 

One You

One You South Gloucestershire  is a free healthy lifestyles and wellbeing service for anyone over 18 who is a South Gloucestershire resident or registered with a South Gloucestershire GP practice.

They can provide telephone or email support to enable patients to build a healthier future.

Whether their goal is to:

They support patients to set achievable goals and help them stay engaged and motivated with one-to-one buddy support

Refer

Please note:

Healthy weight support: to refer patients into the Tier 2 adult weight management programme please complete the ‘South Glos Weight Management on Referral Form’ via EMIS.

Quit Smoking Service: Please use your in-house quit smoking service before referring patients to One You

Health and Happiness Hubs

Health & Happiness hubs are a collaboration between South Gloucestershire Council, GP surgeries, South Gloucestershire Locality Partnership and Southern Brooks. They are for people with a long-term health condition who want to make positive changes to improve their health and happiness.

Eligibility

  • Registered with a South Gloucestershire GP Practice
  • Want to make positive changes to improve your health and happiness
  • Have one or more long-term health conditions, which may include:
    • Hypertension / high blood pressure
    • Type 2 diabetes or pre-diabetes
    • Issues with overweight
    • Chronic pain or painful conditions (inc. fibromyalgia)
    • Anxiety or mild depression
    • Chronic kidney disease
    • Long COVID
    • Coronary heart disease
