Self- referral - Badger Notes

Patients can now use Badger Notes to Self-refer their pregnancy in BNSSG

Antenatal Booking

Patients can use the link above if they are pregnant and wish to refer for antenatal maternity care at either NBT (Southmead or Cossham) or UHBW (St Michael's or Weston General Hospital). Patients should complete the self-referral form on the link and a member of the team will be in touch to book their first appointment with the community midwife. They recommend patients complete the form as soon as they know they are pregnant; it should take approximately 10 minutes to complete. If a patient needs help completing the form, they can contact the booking team Monday to Friday 8am – 4pm on 0117 414 6743 (NBT) or 0117 342 0850 (UHBW).

Please note:  with effect from 1st July 2024, all maternity blood bookings will include an opt out Hepatitis C test. This is initially a two year programme, to identify mothers who potentially will require Hep C treatment post delivery if the test is positive, and to reduce the risk of mother to baby transmission during delivery. Existing children of mothers found to have an active infection will be offered testing through the paediatric infectious diseases team at UHBW. No treatment for Hepatitis C will be possible during pregnancy or whilst breastfeeding. Children over 18 months can receive treatment through the paediatric infectious diseases team. Referrals

For patients who are booked for delivery under the care of St Michael's Hospital, GPs should send referrals via email to:

obstetricmedicalsecretaries@uhbw.nhs.uk

(referral route using eRS has been closed - December 2023)

Indications for referral include:

  • Pregnancy with a history of medical conditions or mental health illness.
  • Pregnancy with a history of problems in previous deliveries.
  • Pre-Pregnancy counselling for women with medical conditions.

A GP Referral is required for confirmation and communication of the exact medical details if the pregnant woman is:

  • Under the care of a hospital other than UHBristol
  • Not under hospital care for the medical disorder
  • Has an undiagnosed or suspected medical disorder, in which case the GP should refer to the appropriate specialty (if not obstetric).

This is not required if  a direct community midwife (CMW) referral is indicated and this will be accepted if the pregnant woman: 

  • Has been under St Michael's medical obstetric team in a previous pregnancy
  • Had pre-pregnancy counselling by St Michael's medical obstetric team
  • Has been seen by UHBW consultant (for the medical disorder in question)
  • Has type 1 or type 2 diabetes.

Medical Disorders in Pregnancy

Women who are pregnant with pre-existing medical conditions or who develop medical conditions during pregnancy should be referred to their local ante-natal clinic.

This can sometimes be done by a midwife in certain circumstances but often a GP referral is required so that information from the GP record can be obtained. See referral sections above.
