Mental health and wellbeing Integrated Network Team (MINT)

Overview

BNSSG MINT is a multi-professional, collaborative approach to deliver personalised care in communities, building a ‘team around me’ for people accessing mental health support.

There are six place-based hubs, one for each locality partnership. MINT champions a culture of joined up, collaborative working, getting people to the right place at the right time, whilst considering the wider determinants of health.

Someone referred to MINT can expect to be contacted by the team within one working week of receiving their information, to arrange an appointment. An assigned MINT link worker will have a support conversation with the person about their needs and goals to co-create a Personal Wellbeing Plan. Together we will find solutions to help people manage their symptoms and improve their wellbeing.

MINT is accessible to anyone aged 18 and over, registered with a BNSSG GP Practice, with provision of transitional care for those aged 16-18 from AWP CAMHS. 

If professionals would like advice and guidance with finding the right pathway for your patient, please consider joining a shared case review meeting.

MINT Referrals

There are currently two referral routes into MINT:

  • Transfers from AWP’s Primary Care Liaison Service (PCLS) and Bristol Mental Health Single Point of Access (SPA)
  • Referrals from General Practices (currently only for Weston, Worle and Villages). See contact details below.

Whilst the hubs grow in resource, please continue to follow usual referral pathways for mental health support.

MINT will contact General Practitioners when supporting one of their patients.

Shared Case Reviews

Shared case review meetings are regular multi-disciplinary and person-centred discussions for professionals seeking guidance about a patient with complex mental health needs.

To discuss a patient, please contact your local hub. Most hubs will ask you to complete a request form with the patient to outline their needs, what matters to them, and record their consent to be discussed

Practitioners should attend the meeting, or send a deputy, to present the case for discussion.

North Somerset MINT Hubs

Weston, Worle and Villages MINT Hub

Weston, Worle and Villages MINT Hub is open for cases referred by General Practices and transferred from PCLS. Shared case reviews are open for all partners on an ad hoc basis.

Woodspring MINT Hub

Woodspring MINT Hub is open for cases transferred from PCLS.

Shared case reviews are open for all partners and occur on a fortnightly basis.

Bristol MINT Hubs

North & West MINT Hub

North & West MINT Hub is open for cases transferred from AWP SPA and shared case reviews are open for all partners on a fortnightly basis.

Inner City & East MINT Hub

Inner City & East MINT Hub is open for cases transferred from AWP SPA and shared case reviews are open for all partners on a fortnightly basis.

South Bristol MINT Hub

South Bristol MINT Hub is open for cases transferred from AWP SPA and shared case reviews are open for all partners on a weekly basis.

South Glos MINT Hub

South Gloucestershire MINT Hub

South Gloucestershire MINT Hub is open for cases transferred from PCLS. Shared case review meetings (known as IPCT meetings) are open for all partners on a monthly basis per PCN.

This guidance does not override the individual responsibility of healthcare professionals to make decisions appropriate to the circumstances of the individual patient, in consultation with the patient and/or guardian or carer, in accordance with the mental capacity act, and informed by the summary of product characteristics of any drugs they are considering. Practitioners are required to perform their duties in accordance with the law and their regulators and nothing in this guidance should be interpreted in a way that would be inconsistent with compliance with those duties.
