ReSPECT Plus

Overview
ReSPECT Plus is the first shared care plan, hosted on an electronic platform, provided by Black Pear. Please see example shared care plan.
It consists of a combined emergency care plan, based on the nationally endorsed ReSPECT (Recommended Summary Plan for Emergency Care and Treatment) which created a personalised recommendation for someone’s clinical care in emergency situation where they are not able to make decisions or express their wishes.
ReSPECT Plus combines ReSPECT with an advance care plan, replacing the Electronic Palliative Care Coordinating System (EPaCCS).
ReSPECT Plus will ensure that all relevant providers involved in the patient’s care can create, read/write or view the latest version of the plan according to their organisation’s access level.
Key points – for patients
- ReSPECT Plus is designed for adults who wish to plan ahead including those:
- living with one serious, or multiple, long term health conditions
- with a clinical Frailty Score (Rockwood) of 7, 8 or 9, (may consider for score of 4, 5, or 6)
- whose health and wellbeing is deteriorating
- with a serious physical health condition
- diagnosed with a life -limiting illness or approaching the end of their life
- The ‘Plus’ advance care plan will include additional sections about the practicalities and planning of personalised care, to ensure patients receive care that reflects their values, goals and preferences during serious, chronic and deteriorating illness.
- It will facilitate advance conversations between GP clinical staff and patients and their family/friends and carers, about preferences for care and end of life choices
- It will ensure the sharing of information and coordination of care with those involved in the care of that patient, to guide decision-making, and delivery of patient-centred, coordinated care.
- The ReSPECT element will ensure patient preferences for treatment and care are clear in an emergency and/or if their condition deteriorates.

- A paper copy of the ReSPECT Plus form can be held with the person of carer, in their home or care setting.
- Existing plans are still valid. We are not asking you to replace the information already on the system, but to replace it with ReSPECT Plus if and when the information is reviewed. Some of the information on these forms will copy over to the new ReSPECT Plus system from information recorded previously in the GP EMIS record.
Key points – technical benefits
- ReSPECT Plus automatically integrates with EMIS, to find out more refer to the Black Pear User Guide and Training video (Please note this video does not contain audio).
- To maximise the flexibility of the ReSPECT Plus digital form across various locations (care homes, patient visits and clinics etc) remote access devices with a secure access to the HSCN network will be able to access the shared care record remotely. All laptops provided by SCWCSU have a remote Virtual Private Network (VPN) solution that allows connection onto the HSCN network so you can access EMIS and the ReSPECT Plus care plan across a standard internet connection using the VPN.
- ReSPECT forms can be signed electronically by entering the clinicians’ professional registration number and name. This will be recognised by the South Western Ambulance Service and Severnside (OOH and NHS 111).
- A summary of the key information in the care plan will be imported into the Adastra system creating a special note and alerting 111 call handlers and out of hours services that a person has serious or life limiting illness and ensuring that person is prioritised and treated appropriately in line with their wishes. This will be implemented from April 22.
- When the ResPECT Plus is updated by Sirona, Hospices and Acutes, GP practices will see the updated ReSPECT form consultation notes appearing in EMIS, with the username Black Pear. This will show ReSPECT consultations undertaken by these other providers and allow this data to be displayed for the first time in the Connecting Care End of Life/ReSPECT view
- The system will automatically send key information to the ambulance services, replacing the unreliable reliance on administrator processes such as email.
- Connecting Care now has 2 document views under the ‘ReSPECT / BNSSG ReSPECT’ folder. One is the summary view that gives all the information that appears in the ReSPECT form, and the other is the full view that displays the whole ReSPECT Plus form
